Bring your application form to a registry agent, along with an accepted photo ID. Sign and date the form in front of the registry agent clerk.

First, save the file to your computer by right‑clicking the link and choosing where to save it.
Find the file using File Explorer (Windows) or Finder (Mac).
Open the file using Adobe Acrobat Reader by right‑clicking the PDF and selecting "Open with Adobe Acrobat Reader."
Adobe Reader is free if you don’t already have it installed.

Bill of Sale

This form records the legal sale of a vehicle in Alberta and provides the required details for the buyer to register the vehicle.

Driver's Abstract Consent

This form gives written consent for an individual’s Alberta driver’s abstract (standard or commercial) to be released to a specific person, employer, or organization for a permitted purpose.

Authorization for Vehicle Services

This form authorizes a licensed Alberta individual or organization to act on an owner’s behalf to complete motor vehicle registration, plate, or parking placard services under the Traffic Safety Act.

Driver's Abstract Statement of Intent

This form is used when a driver’s abstract is being requested or received by someone other than the driver, confirming the legal purpose and responsibility for handling the information.
